From the dictionary of unusual sex:

Gukuna imishino – Traditional form of >body modification among the Rwandese women, consisting in the elongation of the inner labia with the purpose of making sexual encounters more pleasurable for both partners. The girls begin this practice in their puberty by pulling their flesh and applying solanum aculeastrum dunal and forbicina pelosa plant salves, until they reach a length of about two inches.

It is said to be a really effective pastime: more specifically, gukuna imishino adepts consider >squirting normal and in several occasions they repelled violently the World Health Organization officers who tried to convince them that it was a horrible form of genital mutilation.
